The services that form the basis of a model which will maximise the range of services and improve access to, and responsiveness of, primary and community care services, particularly making available more specialised services nearer to where people live and work.

 

This includes provision of an enhanced diagnostic and treatment capability where appropriate.

Service to enable local people and community groups to access information and space to support a healthier lifestyle. The infrastructure could provide access to training rooms, social space, and information and have extended opening hours.
